The hospice at home team work as part of the Derian House care team and are familiar faces. They link with local children’s community nurses and doctors to ensure care and support is co-ordinated and planned. The service understands that some children, due to their condition, are often poorly even though they may not be in the end stages of life. In this instance nurse support at home may be most suitable.
